Make your reassessment easier with clear evidence, updated goals, and a simple plan for what you want to change.

Quick Checklist

  • Updated goals and what you achieved since the last plan
  • Therapy and support reports (progress, recommendations, hours needed)
  • Roster/schedule showing how supports are used (incl. travel & cancellations)
  • Budget summary (what’s underspent/overspent and why)
  • Quotes for new items/services (if requesting changes)
  • Risk or change in circumstances statements (if applicable)
Show Outcomes

Give specific examples (school, work, community, daily living). Brief dot points beat long prose. Link each example to a goal and the supports that helped.

Evidence Bundle

Collect recent reports (OT, physio, psychology, behaviour support), attendance/usage logs, and relevant quotes. Put them in one PDF if possible.

Requesting Changes

  • Explain what’s changing (needs, risks, life stage) and why current budgets no longer fit.
  • Attach professional recommendations and quotes for new or adjusted supports.
  • Be clear about frequency and duration (e.g., “OT weekly for 12 months”).

Tips for the Meeting

  • Bring a brief agenda (goals, what worked, what didn’t, changes requested).
  • Ask how outcomes will be measured and confirm next steps and timeframes.
  • Take notes or ask your coordinator/plan manager to attend.
