What Is Support Coordination?

It’s a Capacity Building support that helps you make the most of your plan. Your coordinator explains budgets, explores options, and connects you with services that fit your needs and preferences.

What Does a Good Support Coordinator Do?

  • Listens to your goals and preferences, then builds a practical plan to achieve them
  • Explains your categories, line items, and what can (and can’t) be claimed
  • Helps you find and compare providers — without pressure or conflicts of interest
  • Coordinates your team (therapists, supports, plan manager) so everyone’s on the same page
  • Monitors your budget and outcomes, and troubleshoots if something isn’t working
  • Builds your confidence so you can exercise more choice and control over time

How Does Funding Work?

  • Usually appears under Capacity Building – Support Coordination
  • May be time-limited or targeted (e.g., during transitions or complex change)
  • You can be NDIA-managed, plan-managed, or self-managed and still receive Support Coordination
  • Specialist Support Coordination is funded when additional expertise is required

How Do I Choose a Coordinator?

Ask these when comparing services:

  • How will you work with my plan manager and therapists?
  • How often will we meet or check in? (and how do we contact you?)
  • How do you manage conflicts of interest?
  • What’s your experience with situations like mine? (e.g., housing, justice, mental health)
  • How will we track outcomes? (goals, milestones, and budget use)

Working Well With Your Coordinator

  • Share your goals and priorities early
  • Agree on a communication plan (how often, which channel)
  • Keep important documents handy (plan, reports, assessments)

If It’s Not Working

You have the right to change coordinators. Check the service agreement notice period, request a copy of your notes and handover, and choose a new provider that better suits your needs.
